Join a healthcare team focused on medication adherence, patient engagement, and chronic disease prevention.
What You'll Do:
- Conduct high-volume outbound patient calls.
Identify barriers to medication adherence (cost, transportation, forgetfulness, education).
- Coordinate refills and connect patients to available resources.
- Escalate clinical concerns to pharmacists when appropriate.
- Document patient interactions accurately across multiple systems.
- Maintain HIPAA compliance at all times.
What We're Looking For:
- 2+ year of experience in pharmacy, healthcare outreach, or call center environments.
- Knowledge of medication refill processes and patient engagement.
- Strong communication, active listening, and documentation skills.
- Comfortable navigating multiple systems and working on a MacBook.
- CPhT preferred but not required.
Bilingual English/Spanish a plus.
