Job Summary
A mission-driven nonprofit organization in Santa Clara, CA is seeking a Customer Service Coordinator to support nutrition program services for older adults and adults with disabilities. In this role, you will serve as a key point of contact for program participants, families, caregivers, service providers, vendors, and community partners. You will support eligibility, enrollment, intake assessments, scheduling, records management, and program-related questions.

Key Responsibilities
- Communicate professionally and compassionately with clients, older adults, adults with disabilities, caregivers, community partners, and service agency personnel by phone, email, and in person.
- Conduct new client intake interviews, complete assessments, determine program eligibility, and schedule follow-up assessment appointments.
- Complete in-home visits to verify continued program participation status and support compliance with program procedures.
- Manage accurate data entry, database updates, hardcopy records, delivery route reconciliation, reports, and program documentation.
- Answer live calls, return voicemail messages, respond to eligibility and service questions, and help manage the program phone queue.
- Support community outreach efforts, coordinate referrals to related services, and collaborate with program leadership to improve intake, assessment, and recordkeeping practices.
Compensation and Benefits
- Pay: $20 to $21 per hour.

Required Qualifications and Skills
- Coordinate customer service, intake, eligibility, and enrollment support for a community nutrition program.
- Conduct client interviews, assessments, appointment scheduling, and in-home verification visits.
- Maintain accurate participant records, database entries, reports, and delivery documentation.
- Support phone queue coverage, referrals, vendor communication, and community outreach activities.
- High school diploma or GED required.
- 1 or more years of experience in a similar field or position required.
- Experience working with vulnerable populations required.
- Strong verbal and written communication skills with the ability to interact sensitively with older adults, adults with disabilities, caregivers, and agency personnel.
- Experience with data entry, database management, recordkeeping, and accurate documentation.
- Proficiency with Microsoft Office.
- Strong time management skills, independent judgment, and the ability to manage assignments within program deadlines.
- Ability to work independently and as part of a collaborative team.
- Self-starter with strong follow-through, professionalism, and attention to detail.
- Valid California Driver’s License required.
- Acceptable motor vehicle driving record required.
Preferred Qualifications
- Experience in nonprofit, community services, senior services, disability services, healthcare support, social services, or meal program environments.
- Experience conducting community outreach or coordinating referrals with outside agencies and service providers.
- Familiarity with client intake processes, eligibility screening, case documentation, or program compliance procedures.
