Job Summary
A mission-driven nonprofit organization is seeking a Community Program Coordinator for a direct hire opportunity in the South Bay. The Community Program Coordinator will serve as a key point of contact for clients, families, community members, and partner agencies. Key Responsibilities
- Communicate professionally and sensitively with clients, older adults, adults with disabilities, families, the general public, and service agency personnel by phone, email, and in person.
- Conduct intake interviews with new clients to complete assessments and determine eligibility and service needs.
- Assist consumers with program enrollment, eligibility questions, documentation, and general program information.
- Market and promote the program to individuals, community organizations, and potential professional partners.
- Maintain accurate client records, intake documentation, and follow-up notes in accordance with program procedures.
- Support strong relationships with community partners, referral sources, and service agencies.
Compensation and Benefits
- Salary range: $42,000 to $48,000 per year.

Required Qualifications and Skills
- Assist clients with eligibility, enrollment, intake, and general program questions.
- Conduct client interviews and complete intake assessments.
- Communicate with clients, community members, and agency partners across phone, email, and in-person channels.
- Support outreach and marketing efforts to increase program awareness in the community.
- 1 to 2 years of experience in customer support, client care, community services, nonprofit programs, social services, or a related field.
- Strong interpersonal and communication skills with the ability to work professionally with diverse populations.
- Compassionate, patient, and respectful approach when supporting older adults and adults with disabilities.
- Ability to gather information, complete intake documentation, and maintain accurate client records.
- Strong organizational skills and attention to detail.
- Comfortable representing a community program to individuals, referral partners, and service agencies.
- Ability to handle sensitive client information with professionalism and discretion.
Preferred Qualifications
- Experience working in a nonprofit, human services, senior services, disability services, or community-based program environment.
- Familiarity with intake processes, eligibility screening, client enrollment, or referral coordination.
- Experience conducting community outreach or building relationships with local agencies and professional partners.
