A mission-driven healthcare services organization in Santa Clara, CA is seeking a Community Outreach Specialist to support adults, families, and caregivers as they navigate health, care management, and community resource options. The Community Outreach Specialist will support Enhanced Care Management services through outreach, engagement, intake, referrals, reporting, and participation in community-based events. Key Responsibilities
- Conduct telephonic and in-person outreach to potential program participants in a variety of community and care settings.
- Build rapport with clients, explain Enhanced Care Management program benefits, review consent forms, and obtain required signatures.
- Complete comprehensive intake and enrollment activities, including receiving referrals and requesting health plan approvals.
- Coordinate referrals to community, social support, and healthcare-related services based on participant needs.
- Maintain accurate documentation, data entry, reporting, audit support, and compliance with confidentiality requirements.
- Participate in community outreach events, presentations, and planned local travel during working hours as needed.
Compensation and Benefits
- Pay rate: $28 to $29 per hour.
- Direct hire opportunity.
- Hybrid schedule after training is complete: 4 days in office and 1 day work from home.
- Planned event travel may occur during working hours during the week for in-person outreach.

Required Qualifications and Skills
- Perform outreach and engagement for Enhanced Care Management participants.
- Complete intake, enrollment, referral, and health plan approval support activities.
- Coordinate access to community and social support services.
- Support accurate reporting, documentation, audits, and compliance activities.
- Bachelor’s degree or 2+ years of equivalent experience.
- 1 to 2 years of case management experience.
- Valid California Driver’s License and ability to drive locally.
- Knowledge of health promotion, care management, and local community resources.
- Strong communication, relationship-building, and organizational skills.
- Ability to work effectively with diverse populations, adults, families, and caregivers.
- Ability to follow confidentiality laws and maintain accurate, timely documentation.
- Ability to perform prolonged sitting and computer work and lift 5 to 10 pounds.
Preferred Qualifications
- Experience with Enhanced Care Management, managed care plans, or healthcare-related community outreach.
- Familiarity with Santa Clara County community resources and social support services.
- Experience participating in community events, presentations, or public-facing outreach activities.
