School Case Manager – Full Time (40 hrs/week)
Pay: $25/hour

We are currently seeking a dedicated and compassionate School Case Manager to support students and families by connecting them with resources and services that promote academic and personal success. This role is ideal for someone who is organized, empathetic, and passionate about helping youth thrive.

Responsibilities
Provide case management services for students and families
Assess student needs and connect them with appropriate community resources
Collaborate with school staff, parents, and community partners
Maintain accurate documentation and case records
Support student success through advocacy, follow-up, and ongoing communication
Assist in developing and monitoring individualized service plans
Minimum Requirements (Required)
Bachelor’s degree in Human Services or related field, including:
Social Work
Psychology
Counseling
Public Health
Human Relations
Higher Education
Student Affairs Administration
Or closely related field
Minimum 2 years of experience in case management or related human services field
Strong communication, organization, and problem-solving skills
Ability to work collaboratively in a school environment
Schedule & Pay
Full time – 40 hours per week
$25/hour
