**Key Responsibilities**
* Welcome and guide individuals seeking assistance with empathy and respect.
* Conduct intake assessments to identify needs and connect clients to appropriate programs.
* Verify Medi-Cal Eligibility in accordance with CalAIM program criteria.
* Manage referrals from community partners and maintain strong collaborative relationships.
* Ensure accurate documentation and timely follow-up to keep clients on track.
* Advocate for clients by coordinating with internal teams and external agencies.
**Qualifications**
* Passion for serving vulnerable populations and advancing social equity.
* Excellent communication and active listening skills.
* Ability to handle sensitive situations with compassion and confidentiality.
* Organizational skills and attention to detail.
* Experience in social services or case management preferred.
