Registered Nurse (RN) Case Manager

About the Opportunity:
All’s Well Florida is partnering with a city government health services client in Fort Pierce to identify a compassionate and experienced Registered Nurse (RN) Case Manager to support its Health & Human Services operations.
This role is integral to coordinating comprehensive patient care across inpatient, outpatient, and community-based settings. The RN Case Manager will work with a diverse patient population, including individuals with psychiatric and behavioral health needs, ensuring continuity of care, quality outcomes, and strong patient advocacy throughout the care continuum.
Position Overview:
The RN Case Manager is responsible for the development, implementation, and evaluation of individualized care plans, utilizing sound clinical judgment and evidence-based practices. This position requires close collaboration with interdisciplinary teams, proactive care coordination, and consistent compliance with clinical and case management standards.
Key Responsibilities:
• Deliver high-quality patient care through individualized care planning, implementation, and evaluation
• Conduct prospective, concurrent, and retrospective reviews to assess medical necessity and cost-effectiveness
• Collaborate with physicians, social workers, behavioral health providers, and other healthcare professionals to coordinate care and discharge planning
• Serve as a strong patient and family advocate across inpatient and outpatient settings
• Perform in-home or hospital evaluations as needed to assess patient status and care needs
• Ensure compliance with all applicable clinical guidelines, regulatory requirements, and case management procedures
• Maintain accurate documentation and case records in accordance with established protocols
Qualifications:
Education-
• Associate’s Degree in Nursing required (Bachelor’s Degree preferred)
Licensure-
• Active and unrestricted Florida Registered Nurse (RN) License required
Certifications-
• Case Management Certification required or ability to obtain within one (1) year
• Basic Life Support (BLS) certification required
Experience-
• Minimum of two (2) years of experience as a Medical Case Manager
• Experience working with behavioral health or psychiatric patient populations strongly preferred
Skills & Competencies-
• Strong organizational, analytical, and critical-thinking skills
• Excellent verbal and written communication abilities
• Proficiency with Microsoft Office Suite
• Ability to work flexible hours, including evenings, weekends, and holidays as needed
Other Requirements-
• Valid Florida Driver’s License
• Reliable transportation for local travel
