Compensation: $75,000 – $95,000 annually (based on experience)

Position Overview
We are seeking a dedicated and experienced Registered Nurse (RN) Case Manager to join the Health & Human Services team. The RN Case Manager will be responsible for ensuring quality patient care through the development, implementation, and evaluation of individualized care plans. This role requires strong clinical judgment, patient advocacy, and coordination across inpatient, outpatient, and community settings.

The RN Case Manager serves as a critical liaison between patients, families, and healthcare providers—ensuring medical necessity, cost-effectiveness, and appropriateness of services. In some cases, the position may require completing in-home or hospital evaluations as needed.

Key Responsibilities
• Develop, implement, and evaluate individualized patient care plans
• Provide case management services for hospital stays and complex outpatient needs
• Conduct prospective, concurrent, and retrospective review of clinical information to ensure medical necessity and cost-effectiveness
• Serve as an advocate for patients and families, ensuring clear communication and care coordination
• Complete in-home or hospital evaluations as deemed appropriate
• Collaborate with healthcare providers to support safe and effective discharge planning
• Maintain accurate and compliant documentation in line with regulatory standards
• Utilize Microsoft Office and other electronic systems to track patient records and care plans
• Work a flexible schedule including evenings, weekends, and holidays as needed

Qualifications
• Associate’s Degree or higher in Nursing (required)
• Current and valid State of Florida Registered Nurse License (required)
• Case Management Certification (required, or ability to obtain within one year of hire)
• Minimum of two (2) years of experience as a Medical Case Manager
• Current Basic Life Support (BLS) Certification (required)
• Valid Florida Driver’s License (required)
• Broad healthcare background with knowledge of current medical standards of practice
• Proficiency in Microsoft Office Suite (Word, Excel, Outlook, etc.)
• Strong organizational, communication, and interpersonal skills
