Job Summary

We are seeking a compassionate and detail-oriented Nutrition Coordinator for a temp-to-hire opportunity in San Jose, CA. This role supports a community-focused nutrition program serving older adults and adults with disabilities by helping participants with eligibility, enrollment, intake assessments, service coordination, and ongoing program support.

The Nutrition Coordinator is an excellent opportunity for someone who enjoys meaningful work, direct client interaction, community outreach, and helping vulnerable populations access important services. Key Responsibilities

- Communicate with clients, older adults, adults with disabilities, service agencies, vendors, and the general public by phone, email, and in person.
- Conduct client intake interviews, complete assessments, determine service needs, and schedule in-home reassessment appointments.
- Maintain accurate participant records through data entry, database updates, hardcopy files, route sheet reconciliation, and required documentation.
- Manage phone queues by answering live calls, returning voicemail messages, and responding to eligibility and service questions.
- Provide referrals to meal services and related community programs to help participants receive coordinated support.
- Assist with community outreach, reporting, recordkeeping improvements, and best practices for intake and assessment procedures.

Compensation and Benefits

- Pay Rate: $19.00 to $20.00 per hour.
- Job Type: Temp-to-Hire.
- Location: San Jose, CA.

Required Qualifications and Skills

- Conduct intake assessments and determine participant eligibility and service needs.
- Coordinate client communication, appointment scheduling, referrals, and follow-up support.
- Maintain accurate records, database entries, reports, and program documentation.
- Support community outreach and collaboration with agencies, vendors, and professional partners.

- High school diploma or GED required.
- Minimum of 1 year of experience in a similar field or position.
- Experience working with vulnerable populations, including older adults or adults with disabilities.
- Strong verbal and written communication skills with the ability to interact professionally and sensitively.
- Proficiency with Microsoft Office and experience with data entry and database management.
- Strong time management skills with the ability to exercise independent judgment.
- Ability to work independently and collaboratively as part of a team.
- Self-starter with strong organization, follow-through, and attention to detail.
- Valid California Driver’s License required.
- Acceptable Motor Vehicle Driving Record required.

Preferred Qualifications

- Experience in social services, nutrition programs, aging services, disability services, public sector programs, or community-based health services.
- Experience conducting in-home visits, client assessments, or eligibility screenings.
- Familiarity with community resources, referral coordination, or meal service programs.
