Growing Non-Profit in the South Bay seeks a Nutrition Care Coordinator to
assist their growing clientele.

In this role, you will be utilizing a comprehensive network of resources to educate, prepare, support, and advocate for all adults, their families, and caregivers

Key responsibilities include:
1) Assisting consumers with eligibility, enrollment, and general questions by professionally and sensitively addressing the needs of older adults and adults with disabilities. The coordinator will also be responsible for marketing the program to individuals and potential professional partners in the community.
2) Effectively communicate with clients, older adults, and adults with disabilities.
3) Conduct interview of new clients to complete intake assessment and determine their need for service.
4) Schedule assessments appointments by phone and conduct in-home visits to verify continued status for program participation.
