Job Summary

We are seeking a detail-oriented Medical Records Clerk for a contract/temporary opportunity in San Marcos, CA. This role is ideal for a healthcare administrative professional with experience handling medical records, EHR systems, document scanning, indexing, and accurate data entry.

The Medical Records Clerk will support the Health Information Management function by reviewing paper and electronic medical records, identifying key information, entering required details into the EHR, and ensuring documents are scanned and filed accurately. Key Responsibilities

- Accurately enter required information from paper and electronic medical records into the EHR system.
- Collect, prepare, scan, index, and electronically file loose documents for medical record completion.
- Apply sound judgment when filing documents to the correct provider and appropriate EHR location.
- Assist patients with basic medical record questions while maintaining professionalism and confidentiality.
- Navigate EHR and legacy systems to review, locate, and update clinical documentation as needed.
- Complete additional medical records and HIM department support duties as assigned.

Required Qualifications and Skills

- Enter key medical record information into the EHR accurately and according to protocol.
- Scan, index, and file loose medical documentation in the correct patient record location.
- Review medical records and identify required information for accurate documentation.
- Support patients, providers, and internal teams with basic medical records questions.

- High school diploma or equivalent required.
- Enrolled in an Associate Health Information Technology program or at least 1 year of experience in a medical office required.
- Outpatient medical records experience is strongly relevant for this role.
- Knowledge of HIPAA, medical terminology, medical record workflows, document types, EHR systems, and health information processes.
- Proficiency with Microsoft Office and Adobe.
- Ability to communicate clearly and professionally in written, verbal, telephone, electronic, and in-person interactions.
- Strong attention to detail, organization, time management, and accuracy in data entry.
- Ability to work independently, exercise good judgment, and build positive working relationships with colleagues.
- Ability to read and interpret documents, follow verbal instructions, and write routine reports and correspondence.
- Ability to sit for extended periods, use hands and arms frequently, view computer screens and written materials, and occasionally lift up to 20 pounds.

Preferred Qualifications

- Experience in an outpatient medical records or Health Information Management setting.
- Familiarity with EHR navigation, legacy information systems, scanning, indexing, and document management workflows.
- Coursework or active enrollment in Health Information Technology or a related healthcare administration program.
