The Care Team Representative is a key member of the care team, responsible for managing patient phone communications, scheduling appointments, and maintaining provider schedules across two clinic locations. This role involves extensive use of electronic health records for managing patient information, coordinating referrals, and supporting clinical data reporting and quality improvement projects. The position requires excellent communication and organizational skills, flexibility to work some evenings and Saturdays, and a patient-centered approach. Experience in outpatient settings and bilingual Spanish/English skills are preferred. The representative also supports general office tasks and helps implement new technologies to enhance clinic efficiency, all while complying with relevant laws and policies. Must have 2+ years in an administrative medical role and fully bilingual Spanish.
