We are recruiting an experienced Licensed Clinical Social Worker!
Job responsibilities include but aren't limited to:
Provide psychotherapy and behavioral health services to assigned recipients.
Ensure active recipient involvement in treatment and recovery planning.
Conduct outreach when recipient participation decreases.
Maintain continuity of care during hospitalizations or institutional placements.
Complete Mental Health Rehabilitation (MHR) assessments, including strengths assessments and integrated summaries.
Develop and update individualized Treatment Plans with the care team.
Align goals, objectives, and interventions with clinical plans.
Oversee coordinated implementation of all services and interventions.
Accurately complete LOCUS/CALOCUS scoring forms in a timely manner.
Convene Service Planning Teams to review and adjust Treatment Plans as needed.
Collaborate with physicians and healthcare professionals to improve client outcomes.
Educate recipients and families about treatment plans and encourage engagement.
Attend all mandatory meetings as required by the Department of Health and Hospitals (DHH).
Maintain professional relationships with team members, staff, and stakeholders.
Qualifications:
Master’s Degree in Social Work from an accredited institution.
Current and valid Licensed Clinical Social Worker (LCSW) certification (as defined under R.S. 37:2701-2718).
Valid driver’s license and access to an insured vehicle (for community-based services).

Salary $58,240
