Position Summary:
The Referral & Intake Coordinator serves as the first point of entry for the program services. The Coordinator will proactively support program referrals and new client intakes. This role is critical in establishing strong communication and trust-based relationships with clients, families, and referral partners, ensuring a positive and informed program entry experience.
The Coordinator actively manages and processes referrals, tracks referral sources, and monitors key referral and intake metrics to support program performance. This role also works closely with the Program Manager to assign clients to the appropriate Community Supports Specialist (CSS) upon initial program enrollment.
Responsibilities include managing the intake process, including initial eligibility verification, documentation collection, and submission of authorization documents to Managed Care Plans (MCPs). The Coordinator ensures a timely, client-centered onboarding experience, facilitates client and family engagement, and collaborates closely with the Community Support Specialists to ensure seamless service coordination. By ensuring a seamless intake and authorization process, this role lays the foundation for successful transitions, program compliance, and positive client outcomes.
JOB RESPONSIBILITIES:
Referral & Intake Navigation Receive and document referrals from SNFs, hospitals, MCPs, families, self-referrals, and/or outreach efforts.
Proactively manage, process and tracks referral and intake metrics to support program performance and optimize client enrollment
Initiate member outreach within 48 hours of referral receipt to explain program benefits and responsibilities.
Serve as the primary point of contact for intake-related inquiries during the initial engagement phase.
Demonstrate empathy, cultural sensitivity, and professionalism when interacting with clients and families from diverse backgrounds.
Eligibility Verification, Authorization & Documentation
Conduct preliminary screening and verify eligibility in accordance with program criteria.
Prepare and submit authorization packets to MCPs for approval.
Track authorization status, proactively follow up with MCPs and referral sources, and communicate updates to clients and families.
Facilitate a warm handoff to the assigned CSS, ensuring complete and accurate transfer of documentation and case information in designated system.
Provide clear onboarding to clients and their families, including an overview of program benefits, expectations, and responsibilities for maintaining eligibility.
Support continuous quality improvement by identifying process gaps and recommending enhancements to intake and authorization workflows.
Ensure accurate intake data, documentation, and communication logs in accordance with CalAIM program standards.
REQUIREMENTS:
• Associate’s or Bachelor’s degree in social work, healthcare administration, public health, or related field.
• Experience with CalAIM Community Supports or managed care programs.
• Knowledge of RCFE/ALF transitions and/or community-based service coordination.
• Experience providing client and/or family education regarding public program benefits.
• Demonstrated ability to collaborate with multi-disciplinary teams to streamline authorization and service coordination processes.
