Join our mission-driven healthcare team as a Bilingual Health Education Specialist. In this role, you will provide patient education, community outreach, care coordination, and wellness support to diverse populations. You will work closely with providers, nurses, social workers, and community partners to improve health outcomes and help patients overcome barriers to care. Qualifications
Associate's Degree and at least 2 years of experience in health education, counseling, community outreach, patient education, or a related field.
Bachelor’s Degree in a healthcare, public health, education, or related field preferred.
Bilingual communication skills in English and Spanish (written and verbal). Current Basic Life Support (BLS) certification.
CPSP Certification or ability to obtain certification within one year of employment.
Willingness to obtain certifications related to Health Education, Family Planning, and Chronic Disease Management.
Strong presentation, communication, organizational, and relationship-building skills.
Ability to work independently while collaborating with multidisciplinary healthcare teams.
Understanding of cultural diversity, community resources, and underserved populations.
Knowledge of preventive healthcare and patient-centered care models.
Computer & Technology Skills
Experience using Electronic Health Records (EHR/EMR) systems preferred.
Ability to learn and use healthcare platforms including:
NextGen
PMS
QSI
Other electronic healthcare systems as required

Responsibilities
Patient Education & Care Coordination
Assess patient and community health education needs and develop educational plans.
Provide health education on topics including:
Diabetes
Asthma
Heart health
Family planning
Pregnancy and prenatal care
Obesity and nutrition
Smoking cessation
HIV/AIDS awareness and prevention
Support high-risk and special population programs, including CPSP, Family PACT, and Every Woman Counts.
Assist patients with referrals, forms, resources, and healthcare navigation.
Conduct follow-up calls after hospital discharge and assist with scheduling follow-up appointments.
Help patients develop self-management skills for chronic conditions.
Community Outreach
Participate in community events, health fairs, outreach programs, and health education presentations.
Build strong relationships with patients, families, healthcare providers, community organizations, and stakeholders.
Promote preventive healthcare and healthy lifestyle practices within the community.
Team Collaboration & Quality Improvement
Collaborate with physicians, nurses, medical assistants, social workers, and care teams to coordinate patient care.
Assist with population health management and care coordination initiatives.
Support quality improvement activities, chart audits, data collection, and reporting requirements.
Maintain educational materials and presentation resources across assigned locations.
Participate in staff meetings, training programs, and organizational initiatives.
Compliance & Documentation
Maintain accurate documentation and program records.
Ensure compliance with HIPAA, state, federal, and organizational requirements.
Participate in Patient-Centered Medical Home (PCMH) initiatives and performance improvement programs.
