Immediate need for Nutrition Coordinator seeking opportunity to make a difference in the community. Client provides adults and their caregivers the tools and services they need to effectively navigate their health and life options. Through a comprehensive network of resources, Client strives to educate, prepare, support, and advocate for all adults, their families, and caregivers within Santa Clara County.
The Nutrition Coordinator is responsible for assisting consumers with eligibility, enrollment, and general questions by professionally and sensitively addressing the needs of older adults and adults with disabilities. The coordinator will also be responsible for marketing the program to individuals and potential professional partners in the community.
Primary Duties and Responsibilities: • Effectively communicate with clients, older adults, adults with disabilities, the general public and service agency personnel by phone, email and in-person. • Conduct interview of new clients to complete intake assessment and determine their need for service. • Schedule assessments appointments by phone and conduct in-home visits to verify continued status for program participation. • Successfully manage assignments to complete program participant assessments in a timely manner that aligns with program requirements and department procedures. • Provide referrals to consumers for other meal services, as needed. • Accurate data entry into required database systems • Create and maintain accurate hardcopy records of all active and inactive participants. • Interact with other agencies and/or contractors. • Conduct community outreach, as needed. • Assist in the maintenance of clear, concise, and accurate reports and records on a weekly or more frequent basis. • Manage the phone queue by answering live telephone calls and returning voice mail messages related to program eligibility and services. • Provide referrals for programs and services provided by Client to ensure program participants receive coordinated support to continue to age well in their home.• Perform all other duties as assigned.
Requirements: • Excellent verbal and written communication skills. • Independent judgement with time management skills. • Ability to interact and communicate effectively and in a sensitive manner with clients, older adults, adults with disabilities, the general public and service agency personnel. • Experience with data entry and database management. • Demonstrated ability to work independently and as part of a team. • Self-starter. Bilingual in English/Spanish, English/Vietnamese, or English/Chinese is highly desirable.

• Microsoft Office proficient. • At minimum, High school diploma or GED. • 1+ years’ experience working in similar field and/or position. • Experience working with vulnerable populations. • Valid California Drivers License. • Acceptable driving record.
