Certified Aging in Place Specialist (CAPS) Case Manager
Location: Winter Park, FL (serving 7 counties in Central Florida)

Position Summary
We are seeking a compassionate and detail-oriented Certified Aging in Place Specialist (CAPS) Case Manager to join our team. This role is ideal for a licensed Occupational Therapist (OT) or Occupational Therapy Assistant (OTA) who is passionate about helping older adults and individuals with disabilities remain safe and independent in their homes.
The CAPS Case Manager will conduct in-home assessments across seven Central Florida counties to identify accessibility challenges, safety risks, and home modification needs (e.g., grab bars, ramps, and other adaptive installations).
You’ll play a key role in ensuring that each client’s home environment supports independence, comfort, and safety.
Key Responsibilities
Conduct comprehensive in-home evaluations to assess accessibility, safety hazards, and fall risks.
Identify and recommend home modifications and adaptive equipment to meet client needs.
Develop and document detailed home assessments and modification specifications.
Follow up on completed modifications to ensure quality, safety, and compliance.
Provide education and training to clients on safe use of adaptive equipment.
Maintain accurate records of all visits, communications, and assessments in the agency’s database.
Collaborate with contractors, vendors, and team members to ensure timely project completion.
Qualifications
Required:
Current Florida license as an Occupational Therapist (OT) or Occupational Therapy Assistant (OTA).
Valid driver’s license and reliable transportation.
Strong technical and computer skills for documentation and reporting.
Excellent communication, problem-solving, and interpersonal skills.
Preferred:
CAPS (Certified Aging in Place Specialist) certification.
If not currently certified, the organization will cover the cost of a 3-day online training.
Additional Information
Travel is required across seven Central Florida counties (approximately 40–50% of work time).
Use of a company van is available, or mileage reimbursement is provided for use of a personal vehicle.
Most clients are older adults (age 62+) and adults with disabilities seeking to remain safely in their homes.
