Case Manager - Certified Aging in Place Specialist (CAPS)
Hourly Rate: $25–$27 per hour
Location: Corporate Office - Winter Park, FL (travel across 7 counties required)
Monday - Friday Schedule

Position Summary
The Certified Aging in Place Specialist (CAPS) Case Manager is responsible for conducting in-home assessments for clients—primarily adults aged 62 and older and individuals with disabilities—to identify accessibility needs and recommend appropriate home modifications. This position plays a key role in promoting client safety, independence, and quality of life by evaluating home environments, assessing functional limitations, and ensuring recommended modifications meet safety and accessibility standards.

This role requires frequent travel (approximately 40–50% of duties) across seven counties in Central Florida to perform home visits, conduct follow-up assessments, and inspect completed home modifications. A valid driver’s license is required. Employees may use a company vehicle or their personal vehicle; mileage reimbursement is provided when using a personal vehicle.

If the candidate does not already hold the Certified Aging in Place (CAPS) credential, the organization will cover the cost of obtaining it. Training is completed through a three-day online program.

Key Responsibilities
Home Assessments & Client Evaluation
Conduct comprehensive in-home assessments to identify accessibility challenges, fall hazards, and safety risks.
Evaluate clients’ functional abilities related to Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
Assess the need for home modifications such as wheelchair ramps, grab bars, shower handles, hallway supports, and other accessibility features.

Recommendations & Documentation
Develop prioritized recommendations for home modifications based on assessment findings.
Complete detailed home assessment reports and specify modification requirements (including exact measurements and placement details).
Document all assessments, case notes, activities, and communications in the Center for Independent Living’s database or designated Occupational Therapy documentation tools.

Follow-Up & Quality Assurance
Conduct follow-up home visits to ensure modifications have been installed correctly and meet safety standards.
Provide education to clients on safe and appropriate use of installed equipment.
Inspect completed modifications and issue assessments or work orders for adjustments or corrections when necessary.

Collaboration & Client Support
Collaborate with internal teams, contractors, and external partners to ensure efficient coordination of services.
Communicate clearly and professionally with clients, caregivers, and service providers.
Maintain accurate records and ensure compliance with organizational policies and safety guidelines.

Required Qualifications
Valid Florida driver’s license with willingness to travel frequently (40–50%).
Strong technical skills, including the ability to document electronically and use assessment tools and software.
Ability to perform detailed home evaluations, take accurate measurements, and assess environmental risks.
Excellent communication, organizational, and analytical skills.
Ability to work independently, manage multiple cases, and maintain timely documentation.
