About the Role:
We are seeking a dedicated Case Manager to join our clients team.
This position plays a vital role in supporting youth and families through comprehensive case management services.
Key Responsibilities
Provide case management services to clients, ensuring individualized care plans.
Coordinate resources and referrals to meet client needs.
Maintain accurate documentation and compliance with agency standards.
Collaborate with internal teams and external partners to support client success.
Qualifications
Bachelor’s Degree (BS) in Social Work, Psychology, or related field (required).
Bilingual in Spanish and English (required).
Minimum 2 year of strong case management experience.
Ability to pass a Live Scan background check. Clean Driving Record
Excellent communication, organizational, and problem-solving skills.
