Job Title: Case Manager
Pay: $22–$25/hour
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Location: Rancho Cucamonga

Job Summary

We are seeking a compassionate and detail-oriented Case Manager to support individuals by coordinating services and resources that improve their overall well-being. This role involves working directly with clients, their families, and community partners to assess needs, develop care plans, and ensure access to essential services such as housing, healthcare, and food resources.

Key Responsibilities
Conduct comprehensive assessments of client needs, including health, social, financial, and psychological factors
Develop and implement individualized care plans in collaboration with clients and support networks
Connect clients with community resources such as housing assistance, food banks, and healthcare providers
Monitor client progress, evaluate outcomes, and adjust care plans as needed
Maintain accurate and timely documentation of all client interactions and services provided
Utilize motivational interviewing techniques to help clients overcome barriers
Collaborate with interdisciplinary teams including healthcare professionals and community organizations
Participate in meetings, trainings, and case conferences as required
Follow all company policies and compliance standards
Qualifications
High School Diploma or GED required
2–3+ years of customer service experience preferred (human services background is a plus)
Experience supporting individuals with disabilities, mental health conditions, or behavioral challenges highly preferred
Strong knowledge of Microsoft Office (Word, Excel, Outlook, PowerPoint)

Excellent organizational and documentation skills

Work Environment & Physical Requirements
Primarily office-based with extended periods of computer work
Travel within the community to meet with clients and attend outreach activities
Ability to lift up to 15 lbs as needed
