About the Role
We are seeking a dedicated and compassionate Case Manager to provide high-quality supportive services and maintain comprehensive client records. This role is responsible for monitoring client progress for extended periods (up to 2 years), delivering program services, and supporting individuals involved in the criminal justice system. The ideal candidate is empathetic, organized, and passionate about helping clients achieve long-term stability and success.


Key Responsibilities
Client Services & Case Management
• Deliver program services and support participants throughout program enrollment
• Conduct comprehensive needs assessments considering physical, mental, emotional, social, and substance abuse needs
• Develop, review, and update Client Management Plans (CMPs)
• Monitor participant progress and coordinate with corrections and clinical staff
• Provide ongoing problem-solving support and transition planning
Documentation & Compliance
• Maintain accurate and timely documentation in client files using CaseTrack software
• Record services provided, employment progress, counseling sessions, referrals, and incident reports
• Compile program statistics and prepare reports as required
• Present client cases in team and clinical review meetings
Counseling & Program Facilitation
• Conduct individual, small group, and large group counseling sessions
• Facilitate workshops such as relapse prevention, life skills, and money management
• Provide aftercare services in accordance with program requirements
Collaboration & Outreach
• Network with community agencies and social service providers to coordinate referrals
• Work closely with supervisors, clinical staff, and casework team members
• Travel locally to provide outreach and client support (65–75% of workday)


Qualifications
Education & Experience
• Bachelor’s degree in social sciences or related field OR
• High school diploma with at least 2 years of experience providing supportive services to criminal justice or related populations
• Preferred: 3 years of case management or counseling experience in social services, criminal justice, or substance abuse programs

Knowledge, Skills & Abilities
• Strong empathy and excellent listening skills
• Knowledge of social services, corrections, mental health, and substance abuse systems
• Excellent communication, documentation, and organizational skills
• Ability to advocate effectively for clients
• Ability to work independently and as part of a team


Work Environment & Physical Requirements
• Combination of office and field-based work
• Ability to sit, stand, walk, and lift up to 25 lbs.
• Frequent interaction with clients, staff, and community partners
