We are seeking a dedicated and compassionate Case Manager – Mental Health to join our team. The Case Manager will be responsible for assessing client needs, developing individualized care plans, and coordinating services to support individuals with mental health challenges. This role requires a strong understanding of mental health disorders, community resources, and case management best practices. Key Responsibilities: Conduct comprehensive assessments to determine clients’ mental health needs. Develop and implement individualized care plans based on client goals and available resources. Coordinate services, including therapy, medical care, housing, employment assistance, and social support. Provide crisis intervention and advocacy on behalf of clients. Monitor and evaluate clients’ progress, adjusting care plans as needed. Collaborate with mental health professionals, medical providers, family members, and community organizations. Maintain accurate and timely documentation of client interactions and case progress. Educate clients and families on available mental health resources and coping strategies. Ensure compliance with all legal, ethical, and regulatory guidelines. Participate in staff meetings, training, and professional development opportunities. Qualifications: Bachelor’s degree in social work, psychology, counseling, or a related field (Master’s preferred). Licensed or certified as required by state regulations (e.g., LCSW, LPC, LMHC, or equivalent preferred). Minimum of [X] years of experience in mental health case management or related field.
