We are currently seeking a compassionate and experienced professional to provide case management services for veterans during a leave of absence coverage. As a Case Manager II, you’ll play a crucial role in helping veterans navigate challenges related to housing, employment, education, financial stability, and health.
Key Responsibilities:
Conduct comprehensive assessments of veterans referred to the program, identifying needs and barriers in areas such as housing, employment, education/training, and financial stability.
Collaborate with VA personnel and community partners to coordinate services for veterans, including mental and physical health services and eligibility documentation.
Facilitate the client intake process, ensuring all eligibility documentation is complete and up to date.
Collaborate with veterans to develop and update Individual Action Plans (IAPs), outlining their goals and progress.
Maintain accurate and confidential client files in accordance with organizational standards.
Meet with clients regularly, documenting progress and updating plans in the required formats (DAP).
Conduct home visits to project-based units or scattered sites to provide case management services and ensure client well-being.
Lead client life skills classes and provide case management services to meet the unique needs of different veteran populations.
Assist clients with job readiness skills, including resume writing, cover letter preparation, and job placement services.
Facilitate job retention, employment barriers, and skills development classes (e.g., computer skills, money management).
Prepare reports and assist with quality management activities and data collection.
Qualifications:
Bachelor’s Degree required (degree in Social Work, Psychology, Counseling, or a related field preferred). Experience may substitute for degree requirements for employees hired prior to 4/9/15.
At least 2 years of experience in social services or a related field preferred.
Experience facilitating support groups is a plus.
Strong oral and written communication skills.
Ability to work independently and as part of a collaborative team.
Proficiency in Microsoft Office and the internet.
Valid driver’s license preferred (must meet company insurance requirements if driving company vehicles).
