Pay: $23.00 – $24.00 per hour Full time
Schedule: Monday–Friday, 8:30 AM – 5:00 PM (with flexibility for occasional evenings)

If you’re passionate about helping others and want a career where every day matters, this role is for you. As a Bilingual Case Coordinator, you’ll be the bridge that connects individuals to the health and social services they need to thrive.

Meet with clients to understand their needs and create personalized care plans.
Help people access community resources, healthcare, and social support programs.
Provide services in-office, through telehealth, and in the field where clients feel most comfortable.


Qualifications:
Bachelor’s degree in a related field.
Fluent in Spanish and English (speaking, reading, writing).
Knowledge of local community resources and willingness to learn more.
Ability to work occasional evenings/weekends when needed.
Bonus points for experience as a Community Health Worker, Peer Support Specialist, or Medical Assistant.
