Below is information regarding the benefits plan offered to our associates classified as non-exempt full-time (30 hours or more per week) AppleOne Federal Service Contract Associates while working on an approved Service Contract Labor Standards, (SCLS), formerly known as the McNamara-O'Hara Service Contract Act of 1965 (SCA). AppleOne will apply an employer paid Health & Welfare contribution for every regular hour worked (up to 40 hours weekly) to the below Aetna Plan. Any leftover monies will be deposited into a 401(k).

Leftover Health & Welfare dollars may be used to cover the cost of premiums for any eligible dependents added to the plan. Premiums will initially be paid by associates through payroll deductions. Monthly reports will calculate the total deductions taken for dependent coverage as well as the amount of Health & Welfare dollars remaining after the Employee Only premium is paid. If the remaining dollars are less than the total paid in deductions, all remaining dollars will be paid as a taxable employer contribution via direct deposit. Employer contributions will not exceed the amount of the deductions in the prior month. If the remaining dollars exceed the deductions taken, any amount beyond the deduction total will be deposited to the 401k account.

Benefits begin on the first day of the month following one full month worked on the approved SCA applicable federally funded assignment as a full-time (30 hours or more per week) non-exempt AppleOne Federal Service Contract Associate. Prior to the first day benefits begin, all monies will be deposited into a 401(k).

Waiving Benefits

If you already have major group medical insurance, (i.e. through a spouse or military), it may be possible to waive our group coverage plan and request 100% of the employer paid Health and Welfare (H&W) contribution continue to be placed into the 401(k).

To make this request, please fax or scan and e-mail a copy of your current major group medical insurance card (front and back) along with the Waiver Request Form indicating you are requesting to decline our group plan benefits and that you wish 100% of your employer paid Health and Welfare (H&W) contribution placed into the 401(k). Please note, if approved, a copy of your current valid major group medical insurance card will need to be sent in at least once a year for verification and compliance. If not approved, you will be automatically enrolled in our “Employee Only” Aetna (Major Medical Health) Plan.

401(k)

Prior to the first day benefits begin, all of the employer paid Health and Welfare (H&W) contribution is placed into a 401(k). You will be notified via email once your account is created. Once your account is created, you should be able to log in and create a user name and password with Empower in order to access your account.

Empower will send you a package in the mail with further information about your investment options. You may log on to the Empower website (below) to check balances, and make revisions to your investment strategy.

Please allow two to four weeks from the end of the Payroll Cycle Calendar month to see the deposit of funds the following month.
